Almost everyone with spina bifida experiences some type of urinary incontinence.  Whether you have a flaccid bladder, or a spastic one, measures need to be taken to make sure the bladder is fully emptied and kidney reflux or infection is avoided.  A good introduction to continence in spina bifida can be found here .

A video urodynamics test will need to be done to asses bladder function.  Ultrasounds will be preformed to check out the kidneys, and medication may be prescribed.  Antibiotics are often taken to prevent infection and medicines such as Ditropan may be introduced when the individual decides to attempt continence.  Even if you have a child in diapers and are not yet concerned about social continence, a program of intermittent catheterization may be recommended to help empty the bladder and prevent infection.
